Independent Medical Evaluations:
Specialty Independent Medical Examinations:
Civil Action IME
INCLUDES: Detailed narrative report to include an Impairment Rating where applicable of up to 15 pages, records preparation, and review (up to 1000 Pages), a comprehensive physical medical examination and Doctor/client phone consultation if warranted.
Fee $1750.00
*Additional Misc. Doctor hours for Civil cases will be billed at the rate of $600 per hour at the Doctor’s discretion.
Records Reviews:
INCLUDES: Records preparation and processing up to 1000 pages and 2 hours of Doctor’s Record Review and/or consultation ($400/hour workers compensation and $600/hour Civil cases)
Fee $1000 Workers Compensation
Fee $1400 Civil Cases
Additional associated fees:
Records preparation and processing each additional 500 pages over 500
Fee $150.00 per 500 pages
*Doctor’s Record Review and/or consultation per hour over the standard 2 hours
Fee ($400 per hour for Worker's Compensation cases and $600 per hour for Civil action Cases)
Miscellaneous Services and Fees:
Worker’s Compensation Cases
Fee: $250.00 for the first ½ hour of testimony and $100.00 for each ¼ hour of testimony thereafter. *For additional clarification please refer to KY WC 803 KAR 25:160 1,3.
Civil Action Cases
Fee: $2000 for the first hour of testimony and $600.00 for each ½ hour of testimony thereafter.
Civil Action Cases/Video Deposition
Fee: $2500 for the first hour of testimony and $600.00 for each ½ hour of testimony thereafter.
IMPORTANT NOTE:
Appointment Cancellation Fee – Fee: $200.00, The Fee shall be applied for “No-Show” patients and cancellations which occur 5 business days or less prior to the scheduled appointment. This Limited notice does not allow us to properly replace the appointment.
*Commonwealth IME reserves the right to cancel any appointment 3 weeks prior to the appointment if the medical records provided at that time are deemed inefficient to complete an accurate and detailed IME and/or KY Form 107.
All Examination appointments, Record Reviews, and Deposition appointments are subject to an administrative fee of $200, which is included in the total fee listed in the above sections.
* The administrative fee may be considered for refund or transfer on a case-by-case basis for cancellations or changes to an appointment that occur 30 days or more prior to the scheduled appointment date.
