INCLUDES: Detailed narrative report to include an Impairment Rating where applicable of up to 20 pages with a comprehensive physical medical examination, Federal Social Security Medical Source form completed, records preparation, and review (up to 500 Pages), and Doctor/client phone consultation if warranted. Fee $1300.00
