Service Description

INCLUDES: Detailed narrative report to include an Impairment Rating where applicable of up to 15 pages, records preparation, and review (up to 1000 Pages), a comprehensive physical medical examination and Doctor/client phone consultation if warranted. Fee $1700.00 *Additional Misc. Doctor hours for Civil cases will be billed at the rate of $600 per hour at the Doctor’s discretion.
