Standard (W/ Post-Concussion Protocol Evaluation): INCLUDES: Detailed narrative report to include an Impairment Rating where applicable of up to 10 pages (excluding survey and test materials), Basic cognative testing may be administered, records preparation, and review (up to 1000 Pages), a comprehensive physical medical examination and Doctor/client phone consultation if warranted. Fee $1850
